RULES & CONDITIONS

• Promotion runs July 1, 2023, through December 31, 2023.
• Orders must be received from Henry Schein between July 1, 2023, and December 31, 2023.
• Henry Schein item codes must be included on invoice.
• Orders must ship from Brewer by Feb. 14, 2024.
• Redemption form must be received by Brewer by February 28, 2024.
• Offer may not be modified or combined with any other incentive or rebate.
• GPO, local contract (LVA) and government sales are not eligible.
• Returns are not permitted.
• Brewer’s standard warranty terms apply.
• Table orders of a minimum of $10,000 are eligible for free dock-to-dock freight shipping. Liftgate, inside delivery and white glove service is available an additional cost.
• Purchase price must be at normal distributor price to be eligible.
