Understanding the $50B CMS Rural Health Transformation Program.

The Centers for Medicare & Medicaid Services (CMS) has opened applications for the Rural Health Transformation (RHT) Program, providing $50 billion over five years to help states and rural communities strengthen healthcare delivery. This initiative supports investments that make care more accessible, safer, and more sustainable from equipment and facility improvements to workforce and technology upgrades.

Who can apply for RHT funding?

Applications are submitted by state governments, not individual hospitals or clinics. However, states can use awarded funds to support local providers, rural hospitals, and community health centers.

What types of projects or purchases are eligible?

The RHT Program supports improvements that advance access, safety, sustainability, workforce development, and technology — including ADA-compliant exam tables, patient safety features, and modern care infrastructure.

How much funding is available for each state?

The program allocates $50 billion nationwide over five years. Half of the total funding will be distributed equally among approved states, while the other half will be distributed at CMS’s discretion to support the most impactful or innovative projects.

How can a provider or facility benefit from the program?

Even though only states apply, rural providers can partner with their state health departments or regional collaboratives to identify funding priorities. Many projects will include equipment and infrastructure upgrades.

What’s the timeline for applications and awards?

State applications are due November 5, 2025, with awards announced by December 31, 2025. Funding extends for five years through 2030
